  • Define: Established project scope and stakeholder alignment; documented the problem through a SIPOC diagram and charter.

  • Measure: Analyzed dispatch error rates using historical logs and baseline metrics; identified variation and error frequency trends.

  • Analyze: Performed root cause analysis using Cause & Effect diagrams, FMEA, and COPQ; isolated breakdowns in documentation handoff and accountability.

  • Improve: Piloted standardized checklists, peer reviews, and visual cues to streamline documentation. Used QFD and DOE to align changes with customer priorities and validate effectiveness.

  • Control: Instituted a formal control plan with SPC charts, audit checklists, SOPs, and dashboards; improved compliance from 65% to 98% and reduced errors by over 60%.

Results:
